# What is an implant-retained overdenture?

Short answer

An implant-retained overdenture is a removable denture that clips onto two or more dental implants. The implants stop the denture from lifting or sliding, so chewing, speaking and confidence improve greatly compared with a conventional denture, while needing fewer implants and less cost than a fixed full-arch bridge. It is removed daily for cleaning.

An **implant-retained overdenture** is a removable full denture that **clips onto dental implants** placed in the jaw. Small attachments on the implants connect to matching parts inside the denture, so it stays in place when you eat, talk and laugh, yet you can still take it out to clean it. For many people without teeth, especially in the lower jaw, it is the most practical upgrade from a conventional denture: it typically needs only **two to four implants**, far less surgery and cost than a fixed full-arch bridge, and it can often use your existing bone without grafting.

This treatment sits at the centre of Prof. Nouran Abdel Nabi's work. Her published research in prosthodontics studies implant-retained overdentures and their designs, so this page aims to explain the topic as completely and clearly as we can, in plain language.

## Why conventional lower dentures are so often a problem

An upper denture is held by suction against the palate and usually stays reasonably stable. A lower denture has no palate to grip. It sits on a narrow ridge of gum, with the tongue and cheeks constantly moving it. As the jawbone shrinks over the years, the ridge becomes flatter and the denture becomes looser. Many people respond by avoiding certain foods, relying on adhesives or not wearing the lower denture at all.

## Implant-retained vs implant-supported: an important distinction

These terms are often used interchangeably, but they describe different designs:

- **Implant-retained overdenture:** the implants mainly hold the denture in place (retention). Biting forces are shared between the implants and the gum. This is typical of two-implant lower overdentures with individual attachments.
- **Implant-supported overdenture:** more implants, often connected by a bar, carry most of the biting force, and the denture rests less on the gum. This usually needs four or more implants.

Both are removable by the patient, which distinguishes them from fixed full-arch bridges such as [All-on-4 or All-on-6](https://elnabidentalclinic.com/en/faq/all-on-4-vs-all-on-6/), which only the dentist can remove.

## Conventional denture, overdenture and fixed bridge compared

| | Conventional denture | Implant-retained overdenture | Fixed full-arch implant bridge |
| --- | --- | --- | --- |
| Implants needed | None | Usually 2 (lower) or 4 (upper) | Usually 4–6 or more |
| Stability when eating | Limited, especially lower | Much improved | Closest to natural teeth |
| Removable for cleaning | Yes | Yes | No, removed only by the dentist |
| Grafting needed | No | Often not | Sometimes |
| Lip and facial support | Good, via the denture flange | Good, via the denture flange | Can be limited if much bone was lost |
| Palate coverage (upper) | Full | Often reduced with 4 implants | None |
| Relative cost | Lowest | Moderate | Highest |
| Routine maintenance | Relines, eventual remake | Attachment inserts, relines | Professional removal and cleaning, repairs |

## How many implants does an overdenture need?

### Lower jaw: two implants is the widely accepted starting point

For the lower jaw, two implants placed in the front, between the areas where the canine teeth used to be, are widely regarded as a minimum standard of care for people without teeth. This was set out in the McGill consensus statement on overdentures in 2002, and later reinforced by the York consensus in 2009. Three or four implants may be chosen when more support is wanted.

### Upper jaw: usually four implants

Bone in the upper jaw is softer, and the forces act differently, so upper overdentures are generally planned on four or more implants, sometimes connected by a bar. One benefit is that the plate covering the palate can often be reduced, which many patients find improves taste and comfort.

## Attachment types: how the denture clips on

The attachment is the connector between the implant and the denture. Each type has its own feel, maintenance needs and indications. Research in prosthodontics, including Prof. Nouran's published work, examines how different attachment designs and implant arrangements behave, because these choices influence stability, how forces reach the bone and the gum, and how much maintenance is needed.

### Stud attachments (ball and low-profile "locator"-type)

Each implant carries its own small attachment, and the denture contains matching caps with replaceable nylon or plastic inserts. They are compact, relatively economical and easy to clean around. The inserts come in different retention strengths and wear gradually, so they are replaced periodically.

### Bar attachments

The implants are connected by a custom metal bar, and clips in the denture grip the bar. Bars splint the implants together and can provide strong retention and support, which is useful in the upper jaw or with softer bone. They need more vertical space, are more expensive, and require careful cleaning under the bar.

### Magnetic and telescopic attachments

Magnets are easy to seat for people with limited hand dexterity but resist sideways movement less. Telescopic (double crown) attachments can be very stable but are technically demanding and costlier.

## Who is a good candidate for an implant overdenture?

- People who have no teeth in one or both jaws and struggle with a loose denture, particularly a lower one.
- Those who want more stability but prefer a removable option they can clean easily.
- Patients with significant bone and gum loss who need the denture flange for lip and facial support.
- People for whom a fixed bridge is not affordable, or who want to avoid extensive grafting.

## Who may not be suitable

Overdentures still involve implant surgery. Uncontrolled diabetes, heavy smoking, recent radiotherapy to the jaw or certain intravenous bone medications need careful assessment and may make a conventional denture the safer choice. People who cannot remove and clean a denture daily, and have no one to help them, may also need a different design. ## The treatment step by step

1. **Assessment:** examination, review of your current denture, X-rays and a 3D scan.
2. **A good denture first:** a worn or poorly fitting denture is usually replaced first, because the overdenture depends on correct tooth position and bite.
3. **Implant placement:** a short procedure under local anaesthetic; your denture is softly relined so you can keep wearing it.
4. **Healing:** typically two to four months, sometimes less in suitable cases.
5. **Connecting the attachments** inside the denture, then reviewing fit and bite.

## Living with an overdenture: care and maintenance

- Remove the denture daily and clean it, the attachments in it and the implant abutments.
- Brush around the implants gently with a soft brush; an interdental brush helps around studs and under bars.
- Expect to replace attachment inserts periodically, often every year or two, as they wear.
- The denture will need relining over time as the gum under it changes.
- Attend regular checks so the implants, gum and bite can be monitored.

This maintenance is a normal part of the treatment, not a sign of failure.

## What it costs, and why

Cost depends on the number of implants, the implant system, the attachment type (bars and telescopic designs cost more than studs), whether a new denture is needed and any grafting. A two-implant lower overdenture generally costs considerably less than a fixed full arch; your exact cost depends on an examination. ## Why we focus on overdentures at El Nabi

In overdentures, careful prosthodontic decisions make a large practical difference: implant position, attachment choice, denture design and bite balance. Prof. Nouran Abdel Nabi's research focuses on exactly these questions, and that perspective shapes how we plan each case at our clinic in Maadi. You can read more about her background on the [Prof. Nouran Abdel Nabi](https://elnabidentalclinic.com/en/dr-nouran-abdel-nabi/) page.

## When to see a dentist

If a loose lower denture is limiting what you eat or how confidently you speak, an overdenture assessment is worthwhile. Often your existing bone in the front of the jaw is enough for two implants. You can message us on WhatsApp to arrange a consultation.
