# What causes bad breath, and how do I get rid of it?

Short answer

About 9 in 10 cases of persistent bad breath start in the mouth, mainly from bacteria on the back of the tongue, gum disease, decay, food trapped around teeth or dentures, and dry mouth. Brushing twice a day, cleaning the tongue and between the teeth daily, and treating gum problems usually fix it. If a clean, healthy mouth still smells, see a doctor to check the throat, sinuses, stomach or blood sugar.

Bad breath (halitosis) is one of those problems people worry about quietly: covering their mouth when they speak, keeping a distance in meetings, or chewing gum all day. You are far from alone, and in most cases there is a clear, treatable cause. **About 90% of persistent bad breath originates in the mouth itself**, not the stomach, according to a review of halitosis diagnosis and management ([Aylıkcı and Çolak, 2013](https://pmc.ncbi.nlm.nih.gov/articles/PMC3633265/)).

The smell mostly comes from sulphur compounds released by bacteria as they break down food debris, dead cells and proteins. Remove the places where those bacteria live and the smell usually goes.

## What causes bad breath?

### Causes in the mouth (the large majority)

- **Coating on the back of the tongue.** The rough surface of the tongue traps bacteria and debris. It is the most common single source, even in people with healthy gums.
- **Gum disease.** Bacteria in deep gum pockets produce a strong odour, often with bleeding gums. See [bleeding gums: causes and treatment](https://elnabidentalclinic.com/en/faq/bleeding-gums-causes-treatment/).
- **Tooth decay, broken fillings and food traps** between teeth or under poorly fitting crowns and bridges.
- **Dentures** that are not cleaned daily or are worn overnight.
- **Dry mouth**, from mouth breathing, certain medicines, dehydration or reduced saliva with age. Saliva naturally washes bacteria away.
- **Infections** such as an abscess, an infected wisdom tooth or a healing extraction site.

### Causes outside the mouth

- **Throat and nose:** tonsillitis, tonsil stones, sinus infections and post-nasal drip.
- **Food, drink and habits:** garlic, onions, spices, coffee, alcohol and smoking.
- **Fasting and crash diets:** during long fasts, including Ramadan, less saliva and fat breakdown can change the breath.
- **Acid reflux.**
- **Medical conditions:** uncontrolled diabetes can cause a fruity or acetone-like breath; kidney and liver disease can also change breath odour, though these are much less common causes.

## Why do I have bad breath when I wake up?

Morning breath is normal. During sleep, saliva flow drops sharply, so bacteria multiply on the tongue and between the teeth. It is usually worse if you sleep with your mouth open, snore, or skip cleaning before bed. If the smell disappears after brushing, cleaning your tongue and having breakfast, it is ordinary morning breath. If it stays strong through the day, look for one of the causes above.

## How to get rid of bad breath at home

The NHS recommends brushing for two minutes at least twice a day, cleaning the tongue daily, cleaning between the teeth daily and keeping dentures clean ([NHS: bad breath](https://www.nhs.uk/conditions/bad-breath/)). A simple daily routine:

1. **Brush teeth and gum line** for two minutes, morning and before bed, with fluoride toothpaste.
2. **Clean your tongue** once a day, from back to front, with a tongue scraper or your toothbrush. Most people are surprised how much this alone helps.
3. **Clean between your teeth** once a day with floss or interdental brushes; food left there is a major odour source.
4. **Take dentures out at night**, brush them and soak them as instructed.
5. **Drink water through the day** and chew sugar-free gum to stimulate saliva, especially if your mouth feels dry.
6. **Use mouthwash as an extra, not a replacement.** Alcohol-heavy mouthwashes can dry the mouth; ask about an antibacterial one if needed.
7. **Stop smoking**, which causes its own odour and worsens gum disease.

## How a dentist treats bad breath

If the smell persists for a few weeks despite good home care, a dental examination is the right first step, because most causes are in the mouth. A typical approach:

| What is checked | Possible finding | Typical treatment |
| --- | --- | --- |
| Gums and pockets | Tartar, gingivitis or periodontitis | Scaling or deep cleaning (see [teeth scaling and cleaning](https://elnabidentalclinic.com/en/faq/teeth-scaling-cleaning/)) |
| Teeth and fillings | Decay, broken fillings, food traps | Fillings or replacing faulty restorations |
| Crowns, bridges and dentures | Gaps under margins, plaque on denture surfaces | Adjusting or remaking the restoration; denture cleaning advice |
| Tongue | Heavy coating | Tongue cleaning routine |
| Saliva | Dry mouth | Hydration, saliva substitutes; review of medicines with your doctor |
| Wisdom teeth and infections | Food trapped under gum flaps, abscess | Cleaning, treatment or extraction as appropriate |

## When bad breath means you should see a doctor

If your dentist finds your mouth healthy and the smell persists, see a physician or ear, nose and throat specialist. Seek medical care sooner if bad breath comes with:

- A sore throat, white spots on the tonsils or fever.
- Blocked nose, facial pressure or post-nasal drip.
- Heartburn, acid taste or difficulty swallowing.
- Excessive thirst, frequent urination, or a fruity breath smell, which can signal high blood sugar and needs prompt medical attention.
- Unexplained weight loss or a mouth ulcer that has not healed in two to three weeks.

### When you are worried but no one else notices

Some people are convinced their breath smells when others do not notice anything. A frank assessment by a dentist, and asking a trusted person, can be reassuring and spares months of unnecessary products.

## How Prof. Nouran approaches bad breath

Bad breath can affect how confidently you speak, smile and sit close to people, and it is often embarrassing to raise. At El Nabi Dental Clinic in Maadi, Prof. Nouran examines the gums, teeth, tongue and any crowns, bridges or dentures, and tells you plainly where the odour is most likely coming from. Because she specialises in prosthodontics, she pays particular attention to old crowns, bridges and dentures, which are common hidden food traps. If the mouth turns out to be healthy, you will be told so and advised which doctor to see next.
